The health visitors are available to help with health matters for under five year olds, such as feeding difficulties and all aspects of child development. They can be contacted on:

District nurses
The district nurses specialise in assessing and providing nursing care to those who are unable to attend the surgery. They will give support and advice to patients and carers and will liaise with other agencies on your behalf. The District Nurses can be contacted on 0333 300 0950.
Intermediate care
Your doctor may refer you to the intermediate care team for assessment if you require a lot of care to manage at home. We have an intermediate care nurse attached to the practice. The aim of this service is to help people stay at home and reduce the need for admissions to hospital. A social worker is also attached to this service.
Social prescriber
The social prescribing service supports you to take control of your own health, make positive lifestyles changes and decide what you would like to do to feel healthier.
The service is here to listen to you and connect you with opportunities in the community that might help you to feel better. This could include supporting you with things like managing your stress, loneliness, finding social groups, learning new skills, physical activity or getting information on employment, benefits, housing and legal advice.

If you are 18 or over, you can ask your clinician to refer you to this service.
